Medical Case Management

Medical Case Management can be defined as promoting optimal recovery and rehabilitation of injured workers in a cost-effective and timely manner. The case manager critically assesses the quality of care, cost and efficiency of medical treatment. Our overall view of Medical Management contains seven major steps:

  • File review and assessment
  • Coordination of medical services in a timely and efficient manner
  • Coordinates and attend appropriate independent medical evaluations and Functional Capacity Evaluations
  • Development of client rehabilitation plan
  • Monitoring of client recovery process and appraising all parties of progress
  • Employer, physician and therapist consultation
  • Obtainment all records, films and test results
  • Expediting cost effective treatment
Vocational Rehabilitation Services

Vocational Rehabilitation is to assist the client in returning back to either the same job, a modified position, or some other gainful employment. Health Connection provides the following vocational services:

  • Personal vocational assessment
  • Vocational testing
  • Vocational planning and counseling
  • Job analysis
  • Labor market survey
  • Job seeking skills training
  • Job placement
  • Create and revise resumes along with assistance with learning appropriate interviewing skills

Health Connection's primary goal is to return clients to their pre-injury job. If that is not successful, our next step is to place them in alternative employment commensurate with their physical limitations. Our Job Placement Specialists selectively identify open jobs and then schedule and attend employment interviews.
