Medical Case Management

Medical case management is defined as a healthcare process wherein a professional helps a patient or client develop a plan that coordinates and integrates the support services that the patient/client needs to optimize the healthcare and psychosocial possible goals and outcomes.[1] The case management process helps the patient and their family navigate through a complicated set of services and supports available within a benefit plan, an organization or institution, and their community. If you or a loved one has been injured and is facing unexpected challenges navigating the complex healthcare field or an extended recovery time, you need a Medical Case Manager. Our role is to liaise with a patient, their family, and the patient's healthcare providers. We do not diagnose patient illnesses. Cognitive Behavioral Therapy Services

Vocational Rehabilitation

Health Connection of Illinois goal is to return our clients to their pre-injury job through vocational rehabilitation services. If that is not successful, our next step is to place them in alternative employment commensurate with their physical limitations. We selectively identify open jobs and then schedule and attend employment interviews. As experts in vocational rehabilitation services, we are qualified by knowledge, experience, education, training or to testify if needed.
